You will be redirected to the SigmaMD Enrollment site.

• First, enter your own (parent/guardian) information and click the button: “I am enrolling others, but not myself.”

• Click “save,” then “add” to enter each child’s information. If you are pre-enrolling a newborn, you can use a placeholder name and today’s date as the DOB (as it won’t let you add a DOB that is in the future). The DOB can be be changed in the future.

• Next, select how often you’d like to be billed, monthly or annually, then click “continue”.

• On the next screen you’ll choose when you would like the membership to start. It can start on the due date if you are pre-enrolling a newborn.

• Next you will enter your payment information, credit card or ACH, and will complete the following forms: Membership Agreement, Financial and Payment Policy, Communication Consent, Notification of Privacy Practices, and Consent to Treatment.

• You will receive an email with information on how to download the SigmaMD app. This is how we will communicate and where you will schedule appointments, view/change billing information, and view your child’s records.

3. After your SigmaMD enrollment is completed, you will receive a Welcome Guide to Vedella Pediatrics. This will contain the private member line. You may also schedule your child’s first intake appointment. There will be an Intake Form and an Authorization to Release Medical Records form attached to this first appointment. You may also link your child’s health records by clicking "Connect to external patient portals" on the SigmaMD app.
